Vacation Bible School Child Registration (August 2026)

August 17-21, 2026 | 9am-1pm | Vacation Bible School (VBS) is a week-long summer outreach at Manhattan Grace Tabernacle (MGT), designed to give kids ages 5–12 a fun and creative space to learn about Jesus and the Bible while building new friendships within the MGT community.

It is imperative for your child(ren)'s safety and well-being that you inform us of any allergies or medical problem they may have. This information will not prevent your child(ren) from participating in the program.

Consent & Medical Release
By way of the registration information, which I have completed, I hereby give permission to my child(ren) to attend Vacation Bible School Monday-Friday, August 18-22, 2025. I certify that my son/daughter is voluntarily participating in this event. I understand that with participation come risks inherent to such activity, transportation and location of the event. I hereby release Manhattan Grace Tabernacle, its sister churches and all employees from any liability for injury, loss or damage to my child(ren) or his/her property, which may result from said participation.

I give permission for photographs of the persons listed here to be used in publications of MGT (including the church website and social media). I understand that these photos may be viewed by anyone but no identifying information will be displayed.

If a medical emergency should arise where my child(ren) should need immediate medical attention other than first aid, I hereby give consent to the leaders and/or the assistant leaders, to take my child(ren) to the nearest hospital and I will be notified immediately as well. Under no circumstances will anyone administer medication at any time to my child(ren).
